In most cases, they experience a dramatic deterioration in functioning and require assistance in strengthening muscle mass, return to daily activities and more. This process, known as deconditioning, is complex and longterm, and the most comfortable way of conducting it successfully is at home, supported by the multidisciplinary medical staff accompanying the patient and his family in the deconditioning process at home, and is available to them 24/7.
A personalized treatment program is devised for every patient. The treatment team comprises a permanent staff - a physician specializing in home rehabilitation, a nurse, a social worker, and a multidisciplinary rehabilitation team as required. Continued support at home will allow these patients to adapt gradually, under medical supervision, and therefore enable a faster return to the normal routine.
What is deconditioning?
“Deconditioning” is for patients who have experienced a deterioration of functioning due to a prolonged admission, are post-surgery or a complex illness, and require rehabilitation to return the functions that were damaged. The damage could be expressed as a decrease in walking ability and independence, in going from lying down to a sitting position, and in other basic functions due to being confined to bed for an extended period, on a background of heart disease, severe infections, complex respiratory illnesses, complex surgeries etc.

How is treatment at home managed following a hospital admission or surgery?
In the first stage, a visit will conducted by a specialist physician in order to devise an appropriate personalized home treatment program for the medical needs of the patient. During the evaluation visit, the medical history will be checked and a medical investigation will be carried out, to include a functional assessment, an evaluation of pain, mental condition, suitability of medicines etc.
The treatment program for the post-operative recovery and rehabilitation process at home will define the treatment objectives, the individual medical professionals who will take care of the patient, and an organized visitation program.
